Job summary

Addus HomeCare in Dover, TN is seeking a Full-Time Registered Nurse (RN) to provide one-on-one, in-home nursing care to patients. You will perform assessments, plan and implement care, and coordinate with physicians and the interdisciplinary team.

The role requires a TN nursing license, graduation from an accredited program, CPR certification, and reliable transportation. Home health experience is preferred; evenings/weekends on-call may be required.

Qualifications

• Graduate from an accredited registered nursing program and licensed in the state.
• At least one year clinical practical nursing experience, preferably in ICU, rehab, or med-surg.
• Home health experience preferred.

Responsibilities

• Provide visits as assigned, including assessment, eligibility, admissions processing, planning, implementation and evaluation.
• Obtain data from patient factors and coordinate intervention with the team.
• Initiate communication with physician and interdisciplinary team for optimal care.
• Update plan of care based on patient needs.
• Perform therapeutic, preventative and rehabilitative nursing procedures.
• Perform pronouncement of death and support family as applicable.
• Maintain accurate patient records and timely care plans.

Schedule:

Regular weekday patient visits with participation in a rotating on-call schedule, including evenings and weekends. Daily hours vary based on patient needs, travel, and documentation.

RN What You’ll Do:

• Provide visits as assigned by the Clinical Services Manager, including the assessment, determination of eligibility, admissions processing, planning, implementation and evaluation phases of the nursing process. Assignments will vary depending on licensure.
• Obtain data and assess necessary information from patient physical, psychological, social and spiritual factors that may impact patient and family's needs and coordinate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician, interdisciplinary team and other agencies for the purpose of coordinating optimal care
• Provide visits to assess the patient's needs and update the plan of care
• Perform therapeutic, preventative and rehabilitative nursing procedures as directed by the attending physician
• Perform clinical pronouncement of death and provide support to family, as applicable to licensure.
• Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely

Qualifications:

• Registered Nurse
  • Graduate from an accredited registered nursing program and licensed in the state
  • At least one (1) year clinical practical nursing experience preferably in intensive care, rehabilitation or medical surgical nursing (preferred)
  • Home health experience preferred
  • Possess and maintain valid CPR certification.
  • Must have reliable transportation, current driver's license and appropriate automobile insurance
