Job summary

Addus HomeCare in Dickson, TN seeks a Full-Time Registered Nurse to deliver one-on-one home health care across Dickson County and surrounding areas. You will perform assessments, care planning, and coordinated interventions with a clinical services team.

As a full-time RN, you’ll contribute to patient recovery and independence through skilled nursing visits, documentation, and collaboration with physicians.

Qualifications

• Graduate from an accredited RN program and licensed in Tennessee (or eligible).
• CPR certification must be current.
• Minimum 1 year of clinical nursing experience; ICU, rehab, or med-surg preferred.
• Must have reliable transportation, a valid driver's license, and appropriate automobile insurance.

Responsibilities

• Provide visits as assigned by the Clinical Services Manager, including assessment, eligibility, admissions processing, planning, implementation and evaluation of the nursing process.
• Gather data from patient and family considering physical, psychological, social and spiritual factors; coordinate with interdisciplinary team.
• Communicate with attending physician and other agencies to coordinate optimal care.
• Assess patient needs during visits and update the plan of care accordingly.
• Perform therapeutic, preventive and rehabilitative nursing procedures as directed by the physician.
• Proceed with clinical pronouncement of death and provide family support as applicable.
• Maintain accurate patient records, charts, and timely care plans.

Education

Graduate from an accredited RN program and licensed in TN

CPR certification

One year of clinical nursing experience (prefer ICU/rehab/med-surg)

Reliable transportation, valid driver's license and insurance

Dickson County, TN and surrounding areas.

Schedule: Regular weekday patient visits with participation in a rotating on-call schedule, including evenings and weekends. Daily hours vary based on patient needs, travel, and documentation.

RN What You’ll Do

• Provide visits as assigned by the Clinical Services Manager, including the assessment, determination of eligibility, admissions processing, planning, implementation and evaluation phases of the nursing process. Assignments will vary depending on licensure.
• Obtain data and assess necessary information from patient physical, psychological, social and spiritual factors that may impact patient and family's needs and coordinate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician, interdisciplinary team and other agencies for the purpose of coordinating optimal care
• Provide visits to assess the patient's needs and update the plan of care
• Perform therapeutic, preventative and rehabilitative nursing procedures as directed by the attending physician
• Perform clinical pronouncement of death and provide support to family, as applicable to licensure.
• Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely

Qualifications

• Registered Nurse
• 1. Graduate from an accredited registered nursing program and licensed in the state
  2. At least one (1) year clinical practical nursing experience preferably in intensive care, rehabilitation or medical surgical nursing (preferred)
  3. Home health experience preferred
  4. Possess and maintain valid CPR certification.
  5. Must have reliable transportation, current driver's license and appropriate automobile insurance
