Job summary

Nuclear Care Partners is seeking a Registered Nurse (RN) to deliver skilled, one-on-one home health care in Idaho Falls, ID. You will work with a nurse-led, mission-driven team focused on patient dignity and outcomes, providing compassionate, patient-centered care directly in patients' homes.

Responsibilities include creating and following care plans, documenting care and observations, and collaborating with interdisciplinary teams while complying with DOL, CMS, and state regulations.

Qualifications

• Active Idaho RN license.
• Current CPR certification.
• Valid driver's license with insured vehicle.
• Home health experience preferred but not required.
• Must pass pre-employment screenings.

• Provide skilled, in-home nursing care to assigned patients.
• Develop and follow individualized care plans.
• Accurately document patient care, observations, and updates.
• Collaborate with interdisciplinary care teams.
• Maintain compliance with DOL, CMS, and state regulatory requirements.

Compensation is based on experience, shift type, and market factors. The listed range reflects entry-level to highly experienced applicants.
