Job summary

Elara Caring in Texas is seeking a Registered Nurse Home Health to join our team delivering compassionate care in patients' homes.

You'll assess, plan and provide nursing care, monitor conditions, educate families, and ensure HIPAA privacy, with travel between patient homes. We offer comprehensive benefits and a supportive work environment.

Qualifications

• RN license from state board with active status.
• Graduate of accredited nursing program (Certificate/ Diploma/ ADN/ BSN).
• Minimum 1 year of clinical nursing experience.
• Home Health experience preferred but not required.

Job Description

• Assessing patients to determine their care needs.
• Completing initial and ongoing patient assessments.
• Protecting patient and staff privacy in accordance with HIPAA.
• Providing nursing care according to the plan of care and physician orders.
• Monitoring patient conditions and reporting changes to the care team.
• Educating patients and families on their care and safety.
• Taking appropriate action when patient conditions change.
• Helping ensure quality, safe, and continuous patient care.

What is Required?

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.
• 1+ year of experience as a nurse in a clinical care setting.
• Home Health experience is preferred, but not required.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
