Job summary

BAYADA Home Health Care is seeking a Registered Nurse (RN) for a full-time Senior Living visits role across Central NJ, delivering one-on-one skilled nursing care to adult and geriatric clients in assisted living, independent living, and senior communities throughout Somerset, Middlesex, Monmouth, and Union Counties, NJ.

You will assess needs, develop care plans, administer medications, and coordinate with physicians and the interdisciplinary team while documenting via BAYADA EMR.

Qualifications

• Current New Jersey RN license.
• Minimum of one year of clinical nursing experience.
• Graduate from an accredited and approved nursing program.
• Home health or senior living experience preferred, but not required.
• Ability to work independently and manage time effectively.
• Experience with electronic medical records (EMR) preferred.

Responsibilities

• Provide skilled nursing visits to clients across designated senior living communities and territories.
• Complete comprehensive nursing assessments and develop individualized plans of care.
• Administer medications, provide wound care, and perform physician-ordered treatments and procedures.
• Monitor client conditions and communicate changes to the Clinical Manager or Client Services Manager.
• Follow, execute, and accurately document physician orders.
• Provide case management and coordinate care with physicians, therapists, and other team members.
• Educate clients and caregivers to promote health, safety, and independence.
• Complete timely and accurate clinical documentation using BAYADA's EMR system.

BAYADA Home Health Care is seeking a Registered Nurse (RN) for a full-time opportunity with our new Central NJ Senior Living team, a Medicare-certified home health office.

This position provides one-on-one skilled nursing care to adult and geriatric clients living in assisted living, independent living, senior congregate living, 55+ communities, and intermediate care facilities serving individuals with intellectual disabilities throughout Somerset, Middlesex, Monmouth, and Union Counties, NJ.

As a BAYADA Registered Nurse, you'll have the opportunity to build meaningful relationships with your clients while providing high-quality clinical care in the place they call home. You'll work independently while being supported by an experienced interdisciplinary team that values your clinical expertise.

This position may also include seeing clients in traditional home health community settings.

Qualifications:

• Current New Jersey Registered Nurse (RN) license.
• Minimum of one year of recent, verifiable clinical nursing experience.
• Medical-surgical experience preferred.
• Graduate of an accredited and approved nursing program.
• Home health or senior living experience preferred, but not required.
• Ability to work independently and effectively manage time.
• Strong communication, interpersonal, organizational, and clinical assessment skills.
• Experience with electronic medical records (EMR) preferred.
• Reliable transportation and ability to travel throughout Somerset, Middlesex, Monmouth, and Union Counties.

Additional Information:

Base Pay, depending upon qualifications, experience, and subject to negotiation:
$65.00 - $75.00 per point ($101,400 - $117,000)
