Job summary

BAYADA Home Health Care in New Jersey is seeking an experienced Registered Nurse for a part-time opportunity providing home health visits through the Gloucester County office.

As an RN on BAYADA's multi-disciplinary team, you will follow care plans, administer medications, perform wound care and assessments, and coordinate with clinicians to help clients stay safely at home.

Qualifications

• Active NJ Registered Nurse licensure.
• Minimum 1 year of clinical (medical/surgical) experience.
• Home health experience preferred but not required.
• Graduation from an approved nursing program.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills and computer literacy with EMR systems.

Responsibilities

• Follow a designated care plan to meet patient needs.
• Make home visits to clients in designated geographic territories.
• Administer medications, wound care, treatments, and procedures.
• Monitor client conditions and report changes to clinical leadership.
• Document doctors' orders and client assessments accurately.
• Coordinate case management and care team communications.

As a home care Registered Nurse, you will be an integral member of a multi-disciplinary health care team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home.

Registered Nurse (RN) Responsibilities:

• Follow a designated care plan in accordance with patient's needs
• Make home visits to clients in designated geographic territories
• Perform assigned duties, including administration of medication, wound care, treatments, and procedures
• Monitor clients' conditions; reporting changes to Clinical or Client Services Manager
• Follow up with, execute, and properly document doctors' orders
• Perform client assessments as necessary
• Case management and coordination
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet

Qualifications for a Registered Nurse (RN):

• A current license as a Registered Nurse in NJ.
• A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
• Prior home care experience strongly preferred, but not required.
• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
• Ability to travel to cases as assigned.

Additional Information:

Base Pay, depending upon qualifications and subject to negotiation:
$50.00-$57.00 per point
