Job summary

Angels Care Home Health in the United States is seeking a Registered Nurse Case Manager to coordinate all aspects of patient home care. You will perform admission visits, determine eligibility for home care services and develop the plan of care with the care team.

• Graduate of an accredited nursing program (diploma/associate/bachelor).
• Current RN license in the state.
• One year RN experience; two years preferred.
• Valid driver's license and reliable transportation.
• CPR certification and hepatitis consent/declination.
• Auto liability insurance and ability to travel to patient homes.

Responsibilities

• Coordinate all aspects of the patient’s home care.
