Elara Caring is seeking a Registered Nurse Home Health to join a team delivering quality care in patients' homes. You will assess needs, provide nursing care, and educate patients and families while adhering to HIPAA and safety standards across multiple visits.

Requires an active RN license and 1+ year of clinical nursing experience; Home Health experience is preferred. A valid driver's license or reliable transportation is needed for travel between patient homes.

Formación

• Current State license as a Registered Nurse.
• Graduate of an accredited nursing program (Certificate/Diploma/Associates/Baccalaureate).
• 1+ year of nursing experience in a clinical setting.
• Home Health experience preferred but not required.

As a Registered Nurse Home Health ,you'llcontribute to our success in the following ways:

• Assessing patients to determine their care needs.

• Completing initial and ongoing patient assessments.

• Providing nursing care according to the plan of care and physician orders.

• Monitoring patient conditions and reporting changes to the care team.

• Taking appropriate action when patient conditions change.

• Helping ensure quality, safe, and continuous patient care.

What is Required?

• Current State license as a Registered Nurse.

• Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.

• 1+ year of experience as a nurse in a clinical care setting.

• Home Health experience is preferred, but not required.

• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment

• Must possess a valid driver’s license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
