CareLinx by Sharecare is a nationwide registry of healthcare professionals providing on-demand telephonic nursing support. As a Remote RN in California, you will conduct outreach, assess chronic conditions, and document findings through the CareLinx app, with shifts that fit your schedule.

We require an active California (and Utah) RN license, bilingual English/Spanish, and at least 2 years of acute care experience. This part-time, W2 role offers flexible hours and professional autonomy.

Qualifications

• Active RN license in CA and Utah; licensure in additional states preferred.
• Bilingual fluency in English and Spanish required.
• Minimum 2 years of clinical experience in Acute Care ED, ICU, Telemetry, Stepdown/Progressive Care Unit.

Responsibilities

• Conduct telephonic outreach to patients/family members to assess for disease exacerbations.
• Collect and document comprehensive clinical history.
• Assess patients for exacerbations of chronic conditions (CHF, COPD, diabetes, dehydration, etc.).
• Interpret electronic dashboards to support clinical decisions.
• Notify PCP and relevant third parties when an exacerbation occurs.
• Document assessments electronically in a timely and accurate manner.
• Submit timesheets on time through the CareLinx app.
• Communicate case updates to Care Advisor.
• Maintain client confidentiality at all times.

WHAT YOU'LL DO

• Conduct telephonic outreach to patients and/or family members to assess for disease exacerbations
• Collect and document comprehensive clinical history
• Assess patients for exacerbations of chronic conditions including CHF, COPD, diabetes, dehydration, and others as needed
• Interpret electronic patient dashboards to support clinical decision-making
• Notify the patient's PCP and relevant third partiesin the event of an exacerbation
• Document all assessments electronically ina timelyandaccuratemanner
• Submit timesheets on time through theCareLinxapp
• Communicate with your Care Advisorregardingany case updates or changes in patient condition
• Maintain client confidentiality at all times

WHAT WE'RE LOOKING FOR

Required Qualifications:

Active RN license in California and Utah; licensure in additionalstates preferred

Bilingual fluency in English and Spanishrequired

Minimum 2 years of clinical experience in one or more of the following: Acute Care ED, ICU, Telemetry, Stepdown/Progressive Care Unit

Experience conducting clinical history and assessments with older or geriatric patients

Access to a smartphone capable of running theCareLinxapp (requiredfor scheduling, documentation, and timesheets)

Must pass a background check
