Job summary

CareLinx by Sharecare is hiring experienced caregivers to provide non-clinical, in-home care to health plan members and private clients in your area.

As an In-Home Caregiver in San Bernardino, CA, you will work on-site in client homes with flexible part-time shifts. You control your schedule through the CareLinx app.

Qualifications

• Minimum 6 months caregiving experience or relevant education/certification (CNA, HHA, MA, or equivalent).
• Access to a smartphone capable of running the CareLinx app for scheduling and timesheets.
• Ability to communicate in English; bilinguals encouraged.
• Reliable transportation to client homes.
• Must pass background check.
• Program-specific training may apply.
• Medical clearance such as TB test or physical exam may be required.
• Driver's license and MVR check if driving shifts are accepted.

Responsibilities

• Assist clients with ADLs: companionship, meals, mobility, bathing, grooming, dressing, reminders (non-clinical), and light housekeeping.
• Complete and document care tasks accurately in the CareLinx mobile app during each shift.
• Monitor and report changes to the clinical team.
• Submit timesheets on time via the CareLinx app.
• Communicate case updates with your CarePartner.
• Maintain patient privacy, dignity, and safety; protect confidentiality.
• Support with transportation, errands, hospital discharge, or telehealth navigation as needed.

WHAT YOU'LL DO

Core responsibilities for all caregivers:

Assistclients with activities of daily living (ADLs): companionship, meal prep, mobility support, bathing/grooming/dressing, medication reminders (non-clinical), and light housekeeping

Complete and document all care tasks accurately in theCareLinxmobile appduringeach shift

Monitor and report changes in a client’s condition or self-care abilities to theCareLinxclinical team

Submit timesheets on time through theCareLinxapp

Communicate with your CarePartnerregardingany case updates or client changes

Ensure patient privacy, dignity, and well-being in a safe and secure environment

Maintain client confidentiality at all times

Additionalshift opportunities (based on client needs):

Transportation and errand support

Hospital discharge escort — safelyaccompanyingmembers home after discharge

Telehealth navigation — helping members connect with virtual doctor visits

WHAT WE'RE LOOKING FOR

Required qualifications:

Minimum 6 months ofcaregiving experience, OR relevant education/certification (CNA, HHA, MA, or equivalent)

Accessto a smartphone capable of running theCareLinxapp (requiredfor scheduling, documentation, and timesheets)

Ability to communicate in English (bilingual caregivers strongly encouraged to apply)

Reliable transportation to and from client homes

Must pass a background check

Program-specific requirements (may apply depending on assignment):

Completion of program-specific training prior to accepting shifts

Medical clearance such as TB test, physical exam, or drug screening

Valid driver's license and MVR check if accepting driving shifts

Additionaldocumentation may berequiredbased on state law or client contract
