Job summary

Bristol Hospice is seeking a dedicated RN Case Manager to provide compassionate, whole-person care to patients and families in Dover-Foxcroft, ME, and surrounding areas. This full-time, day-shift role offers a supportive, mission-driven environment with a focus on advocating for patients.

Responsibilities include developing care plans, administering medications, and coordinating with interdisciplinary teams to ensure comfort, dignity, and timely care, while maintaining detailed clinical notes

Qualifications

• Must hold a valid Registered Nurse license in good standing in the state of practice.

• Assess physical, emotional, and spiritual needs throughout the care journey.

• Communicate patient changes to the team for timely responses.

• Conduct thorough assessments of physical, emotional, and spiritual needs throughout the care journey
• Accurately monitor and document vital signs including temperature, pulse, respiration, and blood pressure
• Administer physician-ordered medications and treatments with skill and empathy
• Educate patients and their families on care techniques and symptom management
• Track and communicate changes in patient condition, ensuring timely and effective responses
• Keep detailed and up-to-date clinical notes to reflect patient progress and quality of care provided
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery
• And contribute to other supportive duties as part of a compassionate, mission-driven team
• And additional duties as assigned

What You\'ll Bring to The Team:

• A heart for hospice care and a commitment to exceptional service
• A valid Registered Nurse license in good standing in the state you will work in
• A valid driver’s license and reliable transportation
• CPR certification (current)
• Strong observational and communication skills—both written and verbal
• A flexible, team-oriented attitude and willingness to support additional duties as needed
