Job summary

Acts is seeking a Well-Being and Registered Dietitian to provide comprehensive nutrition, clinical, and lifestyle coaching to residents across care levels. Travel between Heron Point of Chestertown, MD and Manor House in Seaford, DE is required, coordinating with an interdisciplinary team to promote health and well-being.

The role ensures delivery of nutrition services in compliance with regulations, supports QA/QPI initiatives, and contributes to continuous improvement in care quality.

Qualifications

• Bachelor degree in nutrition with Registered Dietitian certification.
• 1-3 years of healthcare, clinical, management, and/or wellness experience.
• Health Coach Certification (must be obtained within six months of employment).
• Current RDN credential with licensure in MD and DE.

Responsibilities

• Provide comprehensive well-being, clinical and lifestyle coaching to residents in all levels of care via visitation, observation, nutritional assessment, medical records documentation, and education.
• Coordinate with an interdisciplinary team to promote community health and well-being.
• Provide consultation to ensure proper delivery of quality nutrition services in compliance with regulations.
• Support QA and Performance Improvement (QAPI) initiatives and department policies.

Acts is currently seeking qualified candidates for our Well-Being and Registered Dietitian position.

This position will require travel between our two communities listed below:

• Heron Point of Chestertown - Chestertown, MD
• Manor House - Seaford, DE

In this role, you will be responsible for providing comprehensive well-being, clinical, and lifestyle coaching services to residents in all levels of care via visitation, observation, nutritional assessment, medical records documentation, and group or individual education. Coordinates with an interdisciplinary team to promote community health and well-being. Provides consultation with federal, state, local, and corporate regulations to ensure proper delivery of quality nutrition services to residents. Supports Quality Assurance and Performance Improvement (QAPI) initiatives and department policies and procedures.

Requirements

The Ideal Candidate Will Meet The Following Requirements

• Bachelors degree in nutrition with Registered Dietitian certification
• 1-3 years of healthcare, clinical, management, and/or wellness experience
• Long-term care experience is preferred
• Current Registered Dietitian Nutritionist (RDN) credential with licensure in
• both* Maryland and Deleware
• Health Coach Certification (must be obtained within six months of employment)

Pay Range

$81,689.00 - $99,985.00 / year. Starting rate will vary based on skills and experience.
