Job summary
Pacific Coast Post-Acute is seeking a knowledgeable, compassionate Registered Dietitian to oversee the nutritional care of residents in our 149-bed skilled nursing facility. You will assess, develop, implement, and monitor individualized nutrition plans as part of an interdisciplinary team to promote recovery and quality of life.
The ideal candidate has experience with skilled nursing regulations, geriatrics, and a passion for improving resident well-being.
Qualifications
- Strong knowledge of skilled nursing and long-term care nutrition regulations.
- Experience with geriatric nutrition and quality improvement initiatives.
- 1–3 years of experience in acute care, skilled nursing, or long-term care.
Responsibilities
- Complete comprehensive nutritional assessments and identify residents at nutritional risk.
- Develop, implement, and monitor individualized nutrition care plans.
- Monitor weight trends, labs, intake, and overall nutritional status.
- Recommend therapeutic diets, supplements, and nutrition interventions.
- Provide nutrition education and counseling to residents, families, and staff.
- Maintain accurate clinical documentation.
