Fouraker Hills Rehab & Nursing Center in Jacksonville, FL, is seeking Registered Dietitians (RD) for full-time and part-time roles in a SNF/long-term care setting. You will assess residents, develop nutrition care plans, and provide counseling to residents, families, and staff.

Work with nursing, therapy, and dietary teams to ensure coordinated care, document progress, and ensure regulatory compliance. Full-time coverage includes two additional facilities; part-time requires 24 hours per week.

Qualifications

• Licensed RD in good standing.
• Bachelor's degree in Nutrition and Dietetics or related field.
• Prior SNF or long-term care experience preferred.
• Strong knowledge of clinical nutrition and therapeutic diets.
• Excellent communication, organization, and teamwork skills.

Responsibilities

• Conduct nutritional assessments for residents in the SNF setting.
• Develop, implement, and update individualized nutrition care plans.
• Provide nutrition counseling and education to residents, families, and staff.
• Collaborate with nursing, therapy, and dietary teams to ensure coordinated care.
• Document assessments, progress notes, and updates according to regulatory standards.
• Ensure compliance with state, federal, and SNF dietary regulations.
• Support food service operations to maintain quality, safety, and resident satisfaction.
• Full-time RD: coverage includes two additional facilities as needed.
• Part-time RD: provide 24 hours per week of dietetic coverage.
