Job summary

Qualifications

Responsibilities

• Assess and evaluate the nutritional needs of residents in a Skilled Nursing Facility (SNF)
• Create, implement, and monitor individualized nutrition care plans
• Provide counseling and education to residents, families, and staff
• Partner with nursing, therapy, and dietary staff to ensure coordinated care
• Document assessments, progress, and updates in medical records; ensure compliance with regulations
• Support food service operations to maintain safety, quality, and satisfaction
