Job summary

PACE Southeast Michigan is seeking a Registered Dietitian to implement the Nutrition Care Process through comprehensive nutrition assessments, diagnoses, and tailored interventions. You will work as part of the interdisciplinary team to provide evidence-based medical nutrition therapy to participants and caregivers with varying levels of nutrition risk.

Qualifications

• Bachelor’s Degree in dietetics, nutrition, or related field.
• ACEND-accredited dietetics program completed.
• Successful completion of the registered dietitian exam.
• Preferred 1 year of experience with aging adults.
• Knowledge of CMS guidelines and PACE SEMI standards.
• Ability to apply adult learning principles for education.
• Analytical skills to identify problems and craft interventions.
• Participation in quality improvement and committees.
• Mandatory in-service training including infection control and BLS.

Responsibilities

• Perform comprehensive nutrition assessments at enrollment and semi-annually.
• Determine nutrition diagnoses and develop evidence-based interventions.
• Document PES statements and rationale in medical records.
• Coordinate care plans with the Interdisciplinary Team.
• Provide individualized nutrition education to participants and caregivers.
• Monitor and evaluate progress toward nutrition goals.
• Participate in Care Plan meetings and share plans with the IDT.
