Job summary

PACE of Southwest Michigan in Niles, MI is seeking a Registered Dietitian to deliver personalized nutrition care to older adults, developing individualized care plans and collaborating with medical teams.

You will conduct assessments, provide MNT for chronic diseases, and support families in the community setting. A $5,000 sign-on bonus accompanies a full-time schedule with generous benefits.

Qualifications

• Bachelor’s or Master’s degree in Nutrition, Dietetics, or related field.
• RD or RDN credential (registered dietitian).
• 1–2 years clinical nutrition experience (geriatrics/long-term care/community health).
• At least 1 year working with frail and elderly populations.
• CDE desired; ServSafe Food Protection Manager certification required within 6 months of hire.

Responsibilities

• Conduct comprehensive nutrition assessments at enrollment and during changes in condition.
• Develop and monitor individualized nutrition care plans.
• Provide medical nutrition therapy for chronic diseases.
• Monitor weight, labs, intake, and nutrition risk indicators.
• Recommend therapeutic diets and oral supplements as needed.
• Participate in IDT meetings and care planning.
• Educate participants and caregivers on nutrition and healthy eating.
• Conduct home visits to assess food access and safety.
• Coordinate with home care teams to address nutrition barriers.
• Organize home-delivered meals and supplements as required.
• Oversee kitchen area and monitor meal services.
