Job summary

BrioHR Pte Ltd is seeking a Registered Dietitian for the PACE of Southwest Michigan program in Niles, MI. The role focuses on planning and delivering individualized nutrition care to older adults to promote health, independence, and quality of life.

As part of the interdisciplinary team, you will assess, plan, implement, and monitor MNT, coordinate with home care teams, and participate in care conferences. A 5k sign-on bonus is offered along with competitive benefits and growth opportunities.

Qualifications

• Bachelor’s or Master’s degree in Nutrition, Dietetics, or related field.
• Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credential.
• 1–2 years of clinical nutrition experience (geriatrics/long-term care preferred).
• One year working with frail and elderly individuals.
• Certified Diabetes Educator (CDE) desired; ServSafe certification required within 6 months of hire.

Responsibilities

• Conduct nutrition assessments at enrollment and periodically.
• Develop, implement and monitor individualized nutrition care plans.
• Provide medical nutrition therapy for chronic diseases.
• Monitor weight trends, labs, intake and risk indicators.
• Recommend therapeutic diets and texture-modified diets.
• Participate in IDT meetings and care planning.
• Provide nutrition education to participants and caregivers.
• Conduct home visits to assess food access and safety.
• Coordinate with home care teams on nutrition barriers.
• Organize home-delivered meals and supplements.
• Oversee the PACE kitchen area and meals monitoring.
