Job summary

Brio Living Services is seeking a Registered Dietitian in St. Joseph, MI, to join the PACE of Southwest Michigan team. This full-time position features a 40-hour week, Mon-Fri, 8am-4:30pm, with a $5,000 sign-on bonus and no weekend or holiday work.

You will craft individualized nutrition care plans and collaborate with a multidisciplinary team to support older adults’ health and independence. Qualifications include RD/RDN credentials, 1–2 years of clinical nutrition experience (geriatrics

Qualifications

• Bachelor’s or Master’s degree in Nutrition, Dietetics, or related field.
• Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credential.
• Minimum of 1-2 years of clinical nutrition experience (geriatrics, long-term care, or community health preferred).
• Minimum of one (1) year working with the frail and elderly.
• Certified Diabetes Educator (CDE), desired.
• ServSafe Food Protection Manager certification required - if certification is not held at the time of hire, it must be obtained within six months of the hire date.
• Valid driver's license and reliable transportation.

Responsibilities

• Conduct comprehensive nutrition assessments upon enrollment, semi and annually, and with any significant change in participant condition.
• Develop, implement, and monitor individualized nutrition care plans.
• Provide medical nutrition therapy (MNT) for chronic diseases.
• Monitor weight trends, laboratory values, oral intake, dentition, and nutritional risk indicators.
• Recommend therapeutic diets, oral nutrition supplements, and texture-modified diets.
• Actively participate in IDT meetings, care planning, and case conferences.
• Provide individualized nutrition education and counseling to participants and caregivers.
• Conduct home visits as needed to assess food access, safety, and ability to prepare meals.
• Coordinate with home care teams to address nutrition-related barriers in the community setting.
• Organize home delivered meals and supplements as required.
• Oversee the PACE kitchen area.
• Assist in monitoring meals including intake, diet compliance, and tray accuracy.
