Job summary
WeCare Centers at Heritage Care Center in Pittsburgh, PA is actively seeking a committed Registered Dietitian for our community. You will ensure the optimal level of quality care for residents, providing medical nutrition therapy and developing nutrition plans.
The role requires at least 1 year in Skilled Nursing Homes or LTC, and you must be a Registered Dietitian. Knowledge of DOH rules and EMR/MDS familiarity will support success in this position.
Qualifications
- Must be a Registered Dietitian.
- Experience in Skilled Nursing Homes or LTC facilities.
- Knowledge of State and Federal DOH rules for SNFs.
- Experience with Nutritional Assessments.
- Experience creating/modifying Nutritional Care Plans.
- Familiar with MDS and EMR for clinical documentation.
- Excellent communication skills.
- ServSafe certification preferred but not required.
Responsibilities
- Provides medical nutrition therapy including nutrition assessment, diet modification, nutrition education, and intervention for the patient population.
- Follows Company Diet Manual and the Clinical Nutrition Policies & Procedures Manual.
- Complies with federal, state, and accrediting standards while preserving confidentiality and HIPAA rights.
- Participates in patient satisfaction programs, departmental meetings, and QA/QI programs.
- Assists Dietary Manager in operations of the dietary department as needed.
