WeCare Centers at Heritage Care Center is seeking a Registered Dietitian to join our team in Pittsburgh, PA. The role focuses on medical nutrition therapy, diet assessment, and care planning to support residents' nutrition needs.

Qualifications

• Must hold valid Registered Dietitian license/certification.
• 1+ year experience in Skilled Nursing Homes or LTC facilities.
• Knowledge of State and Federal DOH rules for SNF.
• Experience performing nutritional assessments.
• Experience creating/modifying nutritional care plans.
• Familiarity with MDS and clinical documentation in EMR.
• Excellent communication skills.
• ServSafe certification preferred but not required.

Responsibilities

• Provide medical nutrition therapy including nutrition assessment, diet modification, education, and intervention.
• Follow Diet Manual and Clinical Nutrition Policies & Procedures.
• Comply with federal/state/accrediting standards and HIPAA policies.
• Participate in patient satisfaction programs, meetings, QAPI programs.
• Assist Dietetic Manager in operations of the dietary department.

WeCare Centers at Heritage Care Center is actively seeking a committed, Registered Dietitian for our community in Pittsburgh, PA. Our Dietitian is responsible to ensure the optimal level of quality care is provided to our residents.

Key Responsibilities

• Provides medical nutrition therapy including nutrition assessment, diet modification, nutrition education, and intervention for the patient population

• Follows all guidelines as outlined within the Company Diet Manual and the Clinical Nutrition Policies & Procedures Manual

• Complies with all regulatory standards to include federal, state, and accrediting agencies while adhering to facility confidentiality, HIPPA regulations, and patient rights policies

• Participates in patient satisfaction programs, departmental meetings, and facility wide Quality Assurance/Performance Improvement programs

• Will assist Dietary Manager in all operations of the dietary department as needed

Skills, Knowledge and Expertise

• At least 1 years' experience in Skilled Nursing Homes or LTC facilities

• Must be a Registered Dietitian.

• Knowledgeable with State and Federal DOH rules and regulations for Skilled Nursing Homes
