Senior Community Care of Michigan PACE, based in Lansing, MI, is seeking a Registered Dietitian to provide comprehensive nutrition care within a PACE program. The role emphasizes medical nutrition therapy for elderly participants, collaboration with an interdisciplinary team, and ongoing assessments on a scheduled cadence.

The position offers a supportive leadership culture, a welcoming campus, and opportunities for career growth, with a schedule of four days per week and competitive hourly pay.

Competenze

• Bachelor’s or advanced degree in food and nutrition or dietetics required.
• Licensed in the state as a Dietitian; RD certification required.
• Minimum of one year of experience with frail or elderly populations.
• Valid driver's license and transportation required.

Mansioni

• Provides quality Nutrition Care and Medical Nutrition Therapy to participants and/or caregivers.
• Performs yearly in‑person reassessment; semi-annual reassessment if active in Plan of Care development.
• Participates as an Interdisciplinary Team member and coordinates care with staff.
• Monitors weights, diagnoses, drug-nutrient interactions, hydration, intake, skin condition; reports changes.
