Job summary

PACS is seeking a Registered Dietitian to provide comprehensive nutritional care in a fast-paced long‑term care setting. You will conduct thorough nutrition assessments, develop interventions for high‑risk residents, and collaborate with an interdisciplinary team to manage complex geriatric needs.

Responsibilities include MDS documentation, menu reviews with the dietary manager, and ensuring CMS‑compliant nutrition practices. State licensure and licensure updates are required.

Qualifications

• Must be a Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) certified by the CDR.
• Hold a current state license or certification as required by practice state.
• 1–2 years of clinical experience in long-term care, sub-acute, or skilled nursing preferred.
• Proficiency in calculating enteral nutrition regimens and recognizing malnutrition.
• Experience in MDS entries and long-term care software.

Responsibilities

• Complete nutrition assessments on admission, quarterly, annually, and changes in status.
• Develop and monitor interventions for high‑risk residents (weight loss, tube feeds, skin integrity, dialysis).
• Participate in weekly interdisciplinary care planning with nursing, SLP, physicians and PTs.
• Ensure timely MDS documentation within the EHR and MDS system.
• Collaborate with the Dietary Manager to review menus and ensure safe, nutritious diets.

Job description

Core Responsibilities & Duties

• Nutritional Assessments: Complete comprehensive nutrition evaluations upon admission, quarterly, annually, and during significant changes in status, utilizing standardized tools (e.g., MDS 3.0 Section K).
• High-Risk Resident Management: Develop, implement, and monitor specialized clinical interventions for high-risk residents, specifically targeting involuntary weight loss, tube feedings (enteral nutrition), altered skin integrity (pressure injuries), and dialysis.
• Interdisciplinary Care Planning: Actively participate in weekly interdisciplinary care team meetings, collaborating with nursing, speech therapy (SLP) for dysphagia management, physicians, and physical therapists.
• MDS Documentation: Ensure timely and accurate documentation of resident nutritional status and interventions within the Electronic Health Record (EHR) and MDS system.
• Food Service Collaboration: Work closely with the Dietary Manager to review menus, ensure liberalized diet plans meet nutritional adequacy, check food palatability, and maintain high standards of sanitation and food safety.
• Regulatory Compliance: Maintain facility readiness for state and federal surveys by ensuring all clinical nutrition care practices align with CMS F-Tags (e.g., F692 for Nutrition/Hydration, F693 for Tube Feeding).

Required Skills & Qualifications

• Active Credential: Must be a Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) certified by the Commission on Dietetic Registration (CDR).
• State Licensure: Hold a current state license or certification as required by the state of practice.
• Experience: Minimum of 1–2 years of clinical experience in a long-term care, sub-acute, or skilled nursing setting preferred. New graduates with strong clinical rotations in geriatrics will be considered.
• Specialized Knowledge: Proficiency in calculating enteral nutrition regimens, identifying signs of malnutrition, and managing complex geriatric disease states.
• Technical Skills: Experience using long-term care software (e.g., PointClickCare, MatrixCare) and executing MDS entries.

Physical Demands & Work Environment

• Setting: Fast-paced skilled nursing and rehabilitation facility.
• Physical Effort: Requires frequent walking, standing, and sitting to conduct resident assessments across multiple units.
