Job summary

Nutrition that Works, LLC is seeking a consultant dietitian to join our team for the long‑term care community in Carmichael, CA. This hybrid role offers 16–40 hours per week with flexible scheduling and paid drive time.

You will perform clinical nutrition assessments, document with the Nutrition Care Process in the EMR, and collaborate with the interdisciplinary care plan team to optimize patient outcomes. Expertise in MDS Section K and wound/nutrition changes is valued.

Qualifications

• CDR credentialed dietitian with active licensure.
• Strong communication and client‑centered approach.
• Experience with clinical nutrition assessments and care planning.
• Familiarity with MDS and wound assessment is a plus.

Responsibilities

• Completing clinical nutrition assessments.
• Documenting using the Nutrition Care Process in the EMR system.
• Collaborating with the interdisciplinary care plan team.
• Completing the MDS - Section K.
• Calculating TF.
• Making recommendations for wounds and significant weight changes.
