Screens and assesses patients in an accurate and timely manner based on levels of care; determines patients' nutritional needs and status and provides follow-up care.
Interviews patients and/or their families to obtain history of food habits and preferences.
Monitors patients for and provides education on food/drug interactions. Records food and drug interactions in the education section.
Interprets pertinent laboratory, physical, and social data in the medical record as it applies to nutrition care.
Provides nutrition education and records the patient's diet, diet instructions, and understanding of the diet in the education section.
Consults with physicians regarding appropriate diet modifications and modifies patient menus as needed.
For Post-Acute locations: New York State Certification issued by the New York State Education Department (NYSED) as a "Dietitian-Nutritionist".
For Acute locations: Registration by the Commission on Dietetic Registration (CDR) as a Registered Dietitian.
Primary Source Verification is required.
Graduates from an accredited college/university with a degree in Dietetics or Foods and Nutrition.
AHA BLS certification is preferred.
A Master's degree is preferred.
Special note for Dietitians assigned to Post-Acute Care (Long-Term Care) facilities: Under the Medicare and Medicaid Programs, all newly hired Dietitians in these facilities are required to have NYSED certification as a "Dietitian-Nutritionist" at the time of hire, as outlined in Section 483.60 of Title 42 of the Code of Federal Regulations.
