Job summary

Healthcare Nursing and Rehab is seeking a Regional Social Worker to provide leadership and guidance to facility-based social service teams across multiple skilled nursing centers. You will support regulatory compliance, discharge planning, and psychosocial care to residents and families.

The role requires strong communication, organizational skills, and ability to travel regionally between facilities. Previous leadership in long-term care and knowledge of survey readiness are preferred.

Qualifications

• BSW required; MSW preferred.
• Current social work licensure as required by PA/NJ.
• Minimum 5 years in long-term care with leadership/multi-site experience.
• Strong knowledge of discharge planning, resident rights, and care coordination.

• Bachelor’s degree in Social Work (BSW) required; Master’s degree (MSW) preferred.

• Strong knowledge of discharge planning, resident rights, care coordination, and documentation standards.
• Excellent communication, organization, and leadership skills.
