Qualifikationen

• BSW required; MSW preferred.
• Current state licensure in PA and NJ.
• Minimum of 5 years in long-term care with leadership or multi-site experience.
• Strong knowledge of discharge planning, resident rights, care coordination, and documentation.
• Excellent communication, organization, and leadership skills.
• Ability to travel regionally between facilities.
• Leadership experience overseeing facility social work teams.

Aufgaben

• Provide regional oversight, mentorship, and training to facility social workers and department leaders.
• Ensure compliance with federal, state, and local regulations governing social services in skilled nursing facilities.
• Support facilities in resident assessments, care planning, and discharge processes.
• Collaborate with clinical, admissions, and administrative teams to promote quality outcomes and smooth transitions of care.
• Conduct on-site visits to monitor department performance and assist with survey readiness.
• Serve as a resource for complex psychosocial cases and family concerns.
• Contribute to the development of company-wide policies and best practices for social services.
