Job summary

West Virginia is seeking a regional nurse leader to oversee medical case management under the Ryan White Part B Program. You will perform comprehensive patient assessments, develop individualized care plans, and monitor clinical indicators to optimize treatment outcomes.

The role ensures clients access primary, specialty, behavioral, dental care and supportive services while maintaining strict documentation and HRSA-compliant records.

Qualifications

• Bachelor’s degree in nursing with 4+ years of RN experience
• Associate degree in nursing with 6+ years of RN experience
• Master’s degree in nursing with current WV license or permit

Responsibilities

• Perform comprehensive patient assessments and develop individualized care plans
• Track key clinical indicators to optimize treatment outcomes
• Ensure clients access primary, specialty, behavioral, dental care and supportive services
• Maintain rigorous documentation and HRSA-compliant records
• Generate regular performance and summary reports for regional services
• Collaborate with healthcare providers, pharmacists, social workers, and community organizations
• Develop and distribute educational materials on HIV care and risk reduction
