Job summary

American Health Plans is seeking a Member Advocate to support local service areas with a focus on customer service, member engagement, and CMS-compliant communications. The role includes travel to partner nursing homes within a defined region and maintaining a productive remote workspace.

Responsibilities include welcoming new members, monthly member visits, building provider relationships, documenting activities, and ensuring member needs are met.

Qualifications

• Experience in healthcare delivery with elderly patients preferred.
• Ability to work independently and with a team in a community-based setting.
• Strong organizational and multitasking abilities.
• Excellent written and verbal communication skills.

Responsibilities

• Welcome new members and onboard them per plan processes.
• Visit or contact assigned members at least monthly.
• Develop relationships with network providers and nursing home staff.
• Support CMS-compliant interactions and document activities.
• Coordinate facility visits and ensure coverage within the region.
