Job summary

American Health Plans seeks a dedicated Member Advocate to coordinate with Insurance Producers and ensure seamless enrollment-to-service transitions. You will serve as primary contact for members and families, addressing questions and providing periodic plan updates.

Responsibilities include frontline support with facilities, care teams, and providers within the assigned region. The role requires travel, excellent communication, and strict CMS compliance adherence.

Qualifications

• Experience in healthcare delivery with elderly patients (clinician, social worker, or caregiver).
• CMS regulation compliance knowledge.
• Licensed driver with reliable transportation.

Responsibilities

• Coordinate with Insurance Producer to ensure smooth transitions post-enrollment.
• Follow up with members and families with questions or plan updates.
• Provide frontline support to members, facilities, and care teams.
• Develop and maintain relationships with network providers/facilities in the region.
• Hold monthly meetings with members and/or families.
