• Strong written and verbal communication; ability to simplify complex issues.

Responsibilities

• Coordinate with Insurance Producer to ensure seamless transition between enrollment and customer service.
• Follow up with members to answer questions and provide plan updates.
• Provide frontline support to members, facilities and care teams to meet needs.
• Build and maintain relationships with network providers within the assigned region.
• Meet with members or families monthly to review status and concerns.
• Ensure all interactions comply with CMS regulations and plan requirements.
• Travel to facilities within the assigned region and maintain transportation readiness.
