Job summary

OPCO Skilled Management in Prescott, AZ is seeking an MDS Consultant to lead, train and review MDS services across facilities, ensuring compliance with federal, state and local regulations while supporting quality care and fiscal accountability. You will train staff on resident assessments, care plans and MDS timelines.

Qualifications

• RN licensed in the state of practice.
• Minimum 5 years in long-term or acute health care.
• Minimum 5 years as an MDS Nurse in long-term or acute care.
• At least 3 years of multi-facility, regional MDS experience.
• Bachelor's or Associate degree in nursing from an accredited program.

Responsibilities

• Consults with MDS Coordinators through visits and policy guidance.
• Trains new MDS Coordinators on assessments and plans of care.
• Trains on the RAI manual and deadlines for MDS submissions.
• Monitors and ensures compliance with nursing standards and regs.
• Inspects facilities for compliance with federal/state/local regulations.
• May serve as interim MDS Coordinator as needed.
• Leads training with Compliance Team to ensure compliant billing.
• Guides facility management toward a clinical-focused business plan.
• Develops metrics and benchmarks for quality of care.
• Advises Clinical Support Team and Nursing leadership to improve resident wellbeing.
• Develops processes to reduce variability and improve safety.
• Implements QM data collection and reporting policies.
• Assesses eligibility, referrals, disease and case management systems.
• Reviews clinic information systems for standardized reporting.
• RAI guidelines followed to align care with Medicare/Medicaid requirements.
• Uses survey data to address priorities defined with partners.
• Ensures regulatory compliance across facilities and company policies.
• Maintains safe, clean and orderly facilities; enforces infection control standards.
• Prepares staff for inspections and corrects issues raised by surveys.
• Participates in Plan of Correction for inspection outcomes.
• Provides 24-hour on-call support to nursing centers.
• Ensures orientation and ongoing training for MDS staff.
• Other duties may be assigned as needed.
