Job summary

OPCO Skilled Management is seeking a Regional MDS Coordinator to lead clinical leadership teams across multiple facilities. You will develop, implement, coordinate and evaluate MDS services in alignment with federal, state and local requirements while supporting quality care and fiscal responsibility through training and program reviews.

The role requires RN licensure, a nursing degree, extensive MDS experience in multi-facility settings, and willingness to travel to Aztec and Albuquerque NM.

Qualifications

• RN license in the state of practice required.
• Associate or Bachelor’s degree from an accredited nursing school required.
• Minimum of five (5) years in long-term or acute health care required.
• Minimum of five (5) years working as an MDS Nurse in long-term or acute health care
• At least three (3) years of multi-facility, regional MDS experience

Responsibilities

• Consults with and provides technical assistance to the MDS Coordinators through visits and interpretation of policies and regulations.
• Trains new MDS Coordinators in resident assessments, care plans, and documentation.
• Trains new MDS Coordinators on the RAI manual and deadlines for MDS completion.
• Observes MDS practices for compliance with standards and regulations.
• Regularly inspects facilities and nursing practices for regulatory compliance.
• May serve as interim MDS Coordinator as needed.
• Lead staff in developing and executing a business plan for facility operations including clinical management.
• Develops metrics and benchmarks for quality of care and evaluates performance.
• Advises Clinical Support Team and DON to maximize resident wellbeing.
• Develops and uses standardized processes to reduce variability and improve care and safety.
• Develops and monitors quality management policies and QM data reporting.
• Assess and enhance eligibility, referrals, case management, and disease management systems.
• Reviews clinic information systems for standardized reporting of quality indicators.
• Ensure RAI guidelines are followed and align with Medicare/Medicaid/OBRA requirements.
• Utilizes survey data to address priority areas with community partners.
• Ensure compliance with all regulations and company policies.
• Oversee facility readiness for inspections and training of staff.
• Assist in Plan of Correction responses and QA follow-ups.
• Provides 24-hour on-call support to nursing centers.
