Job summary

Signature HealthCARE LLC is seeking an experienced MDS Leader who can travel across multiple sites to support RAI, PPS and quality measures within a long-term care environment. The role requires RN licensure and a minimum of five years of related nursing experience in LTC, with recent MDS experience in the last two years.

The position involves 80–90% travel, including overnight stays. A strong background in staff training, audits, and stakeholder education is essential for success in this role.

Responsibilities

• Assist with development and implementation of RAI, PPS, QM measures, Medicaid Casemix and Medicare/Medi-Cal guidelines for SHC stakeholders.
• Design and present RAI, PPS and Medicare education using media, technology, workshops, and one-on-one sessions.
• Conduct facility site visits to orient new MDS Coordinators and reinforce RAI/MDS/PPS processes.
