Job summary

Professional Case Management seeks a proactive sales and outreach professional to cultivate referral relationships with local hospitals and physician offices, coordinating education events and community outreach. You will generate referrals, manage a CRM, and support PCM’s in-home nursing services.

Travel locally/regionally is required and experience in hospice/outreach is helpful. The role emphasizes compassionate client engagement, driving referrals, and collaboration with PCM Case Managers

Qualifications

• Bachelor’s degree from an accredited college or university, or comparable relevant experience.
• Sales experience required, including ability to cultivate professional referral relationships.
• Knowledge of medical terminology and/or general medical background helpful.
• Computer skills using Microsoft Office products.

Responsibilities

• Create and maintain relationships through regular visits with local hospitals and physician offices, with direct contact with Discharge Planners, Social Workers, and other clinical staff.
• Schedule and participate in regular educational in-service trainings, community visits, luncheons, town and union hall meetings, and marketing events.
• Generate client referrals and track the lifecycle of referrals in company database.
• Maintain, replenish and distribute printed collateral and other marketing materials at primary referral source locations.
• Proactively contact potential clients and initiate possible client referrals via phone and database tools.
• Conduct medical outreach education to doctors’ offices, hospitals, and senior groups; welcome existing PCM clients to service.
• Other duties as assigned.
