Vanderbilt Health seeks a Clinical Pharmacist to provide leadership in a specialized healthcare team and optimize patient drug therapy. You will develop pharmacotherapeutic goals, monitor plans, and lead efforts to prevent medication errors and ADRs.

The role collaborates with Vanderbilt Health Regional Hospital teams to establish standard processes for anticoagulation, antimicrobial, opioid stewardship and diabetes oversight.

Formación

• PharmD degree required and licensed pharmacist in Tennessee or eligible.
• Experience with anticoagulation and antimicrobial stewardship is preferred.
• Strong leadership and collaboration skills.

Responsabilidades

• Lead a specialized clinical pharmacy team to optimize drug therapy.
• Develop pharmacotherapeutic goals and monitoring plans for patients.
• Lead efforts to prevent and report medication errors and ADRs.
• Collaborate with Vanderbilt Health teams to support stewardship programs.
