Job summary

Vanderbilt Health in Nashville seeks a Clinical Pharmacist to lead a specialized healthcare team and optimize drug therapy for adult medicine patients, participating in interdisciplinary rounds and transitions of care.

You will help prevent medication errors, provide drug information to providers and patients, supervise support staff, and contribute to antimicrobial stewardship; relocation assistance is available, and a holiday and weekend rotation applies.

Qualifications

Completion of an ASHP-accredited PGY2 residency or other post-graduate training in internal medicine.
Completion of an ASHP-accredited PGY1 residency.
Equivalent years of experience in internal medicine or relevant clinical setting.
Board of Pharmacy Specialties in pharmacotherapy or related certification.

Responsibilities

Reviews drug orders to ensure safe, appropriate and cost-effective use of medications.
Prepares and dispenses medications; provides drug information and manages therapy per policy.
Participates in interdisciplinary rounds; supports transitions of care and patient education.
Supervises assigned personnel and verifies accuracy of medications before dispensing.
