Job summary

Vanderbilt Health seeks a Clinical Pharmacist to provide leadership in a specialized healthcare team and optimize patient drug therapy. You will develop pharmacotherapeutic goals, monitor plans, and lead efforts to prevent medication errors and ADRs.

The role collaborates with Vanderbilt Health Regional Hospital teams to establish standard processes for anticoagulation, antimicrobial, opioid stewardship and diabetes oversight.

Qualifications

• PharmD degree required and licensed pharmacist in Tennessee or eligible.
• Experience with anticoagulation and antimicrobial stewardship is preferred.
• Strong leadership and collaboration skills.

Responsibilities

• Lead a specialized clinical pharmacy team to optimize drug therapy.

Job Summary:

Provides leadership in a specialized healthcare team and the patient to assure optimal cost‑effective drug therapy. This includes developing specific pharmacotherapeutic goals, recommending a regimen and developing a monitoring plan to meet the individualized drug therapy needs of the patient. The Clinical Pharmacist is responsible for leading efforts in the prevention and reporting of medication errors and adverse drug reactions according to policy and works to continuously improve process and practice in the institution to enhance safety of medication therapy.
The Clinical Pharmacist will collaborate with Vanderbilt Health Regional Hospital teams to establish and maintain standard processes supporting anticoagulation stewardship, antimicrobial stewardship, opioid stewardship, and diabetes and glycemic agent oversight.
