Job summary

MTM Health is seeking a Regional Care Coordinator in Newport News, VA, to manage member transportation from intake to completion. This hybrid role requires three on-site days per week and close collaboration with health plan clients and providers.

Responsibilities include trip monitoring, issue resolution, and maintaining accurate documentation while upholding MTM standards for reliability, timeliness, and quality of care.

Qualifications

• High school diploma or GED (or equivalent).
• 9 months of experience in a Customer Care Representative role within MTM's Contact Center or similar health care case management experience.

Responsibilities

• Coordinate transportation experiences for members from intake through completion.
• Provide trip monitoring, confirmations, and issue resolution to ensure on-time transport.
• Maintain documentation in intake systems and communicate with health plan clients and providers.
