Job summary

Qualifications

• High school diploma or GED completed.
• At least 9 months in Customer Care or health care case management.
• Proficient in Microsoft Office and data entry.

Responsibilities

• Act as liaison with health plan clients, members, and partners to coordinate a superior member experience.
• Provide member support based on client-identified population.
• Monitor and recover trips, verify transportation details, and document in intake systems.
• Communicate professionally and maintain confidentiality.
• Maintain a list of impacted members and follow up to ensure timely transportation.
• Escalate issues and coordinate care with internal teams.
• Understand MTM operations to better service customers and colleagues.
• Process escalated trips to prevent complaints.
• Report issues to local or corporate leadership for prompt resolution.
• Maintain knowledge of MTM technology and portals.
• Adhere to KPIs and monthly development sessions.
• Other duties as assigned.
