Job summary

Eventus WholeHealth seeks a Behavioral Health Support Specialist to assist residents of long term care facilities under licensed supervision. You will conduct assessments, educate patients, and support self-management, with regional travel required.

Role includes coordinating with families, facility staff, and other providers; developing goals and treatment plans; and maintaining timely documentation as an active member of the behavioral health team.

Qualifications

• Bachelor’s degree in Counseling, Social Work, Psychology or related field.
• Experience in a psychiatric or long term care setting preferred.
• Customer service experience preferred.
• Ability to work independently and proactively in long term care.

Responsibilities

• Conduct assessments of behavioral and social needs of patients.
• Meet with families and facility staff to coordinate care.
• Educate patients on clinical conditions and support self-management.
• Provide outreach for treatment adherence and coordinate with teams.
• Develop individual goals and behavioral health plans.
• Coordinate with other agencies and community providers.
• Maintain timely documentation and participate as a team member.
