Job summary

Extended Care Specialists INC is seeking a Behavioral Health Support Specialist to provide behavioral health support to residents of long-term care facilities under supervision of a licensed professional. You may receive training in assessments and interventions aligned with individual care plans, and regional travel will be required.

The role emphasizes collaboration with families and facility staff, documentation, and active participation in the behavioral health treatment team.

Qualifications

• Experience in psychiatric or long term care setting preferred.
• Customer service experience preferred.
• Ability and willingness to function independently and proactively in a long term care setting.
• Effective use of electronic medical records and Microsoft software (teams, excel spreadsheets, etc).
• Skilled in written and verbal communication.
• Skilled in exercising a high degree of initiative, judgment and discretion.
• Ability to handle confidential and sensitive information.

Responsibilities

• Conduct assessments that include the behavioral and social needs of the patients
• Meet with families and facility staff to coordinate care as recommended by licensed professionals
• Assess the patient’s knowledge of their clinical condition and provide education and self-management support
• Provide outreach to clients who are having problems with treatment adherence
• Assist in the development of the individual goals and behavioral health treatment plan
• Coordinate with other agencies and community providers
• Maintain timely documentation of work
• Serve as an active member of the behavioral health treatment team
