Qualifications

• Bachelor’s degree in Counseling, Social Work, Psychology or related field.
• Experience in a psychiatric or long term care setting preferred.
• Customer service experience preferred.
• Ability and willingness to function independently in a long term care setting.
• Proficient use of electronic medical records and Microsoft software.

Responsibilities

• Conduct assessments of behavioral and social needs of patients.
• Meet with families and facility staff to coordinate care.
• Assess patient knowledge of their clinical condition and provide education and self-management support.
• Provide outreach to clients with treatment adherence issues.
• Assist in developing individual goals and behavioral health treatment plans.
• Coordinate with other agencies and community providers.
• Maintain timely documentation of work.
• Serve as an active member of the behavioral health treatment team.
