Job summary

Gulf Coast JFCS in Miami seeks a dedicated professional to coordinate access to medical, legal, mental health, educational, and social services for children and families. You will assist with enrollment, service planning, and ongoing case management, while coordinating transportation and community outreach.

The role emphasizes trauma-informed practice, collaboration with partners, and robust documentation to meet funder requirements. Excellent bilingual communication is highly valued.

Qualifications

• High school diploma or equivalent; a Bachelor’s degree in social services or related field preferred.
• Two (2) years of relevant experience or combination of education and life experience.
• Bilingual, preferably Spanish or Haitian Creole.
• Experience with refugees and immigrants preferred.
• Ability to handle stressful client situations and trauma exposure.
• Professional, courteous interaction with clients, partners and colleagues.
• Knowledge of community resources and strong communication skills.
• Able to advocate and liaise with health and social service systems.
• Proficient in Microsoft Office and data-tracking systems.
• Combination of office and field time within service area.

Responsibilities

• Determine needs for medical, legal, mental health, educational, and social services.
• Assist clients with enrolling in services per their service plans.
• Enroll eligible clients by completing required forms.
• Provide orientation to newly enrolled clients.
• Help clients develop individualized service plans to achieve self-sufficiency.
• Conduct quarterly assessments of service plans.
• Provide case management to enrolled clients.
• Assess transportation needs and arrange transport when needed.
• Accompany clients to appointments.
• Maintain client case files per funder regulations.
• Conduct community outreach and mental health first aid using trauma-informed principles.
• Prepare and document financial assistance provided toward client goals.
• Complete funder and agency forms.
• Ensure grant guidelines are followed and deadlines met.
• Perform other duties as assigned.
