Aufgaben

• Provides intensive case management including assessment, cultural orientation and client follow-up.
• Facilitates and coordinates group programming activities to meet client needs.
• Document and maintain client records across data systems in a timely manner.
• Connect clients to external/internal resources to meet individual goals.
• Collaborate with program manager to ensure timely reporting (monthly/quarterly/annual).
• Maintain professional knowledge through trainings and conferences.
