Job summary

Samaritas in Detroit seeks an Intensive Case Manager to support refugees and eligible populations who have arrived in the United States within the last five years. You will provide comprehensive, individualized assistance across housing, healthcare, education, and other essential services.

The role requires a bachelor's degree in social work or related field, experience with diverse groups, a valid driver's license, and ability to travel locally.

Qualifications

• Bachelor's degree in social work, psychology, public health, or related field.
• Experience working with refugees or vulnerable populations.
• Valid driver's license with a clean driving record.

Responsibilities

• Provide intensive case management services addressing housing, healthcare, education, and specialized client challenges.
• Develop individualized, holistic service plans based on client needs and goals.
• Coordinate client appointments, transportation, and schedules in collaboration with internal teams and external partners.
• Conduct RHS-15 mental health screenings and coordinate appropriate referrals.
• Build and maintain relationships with community service providers and develop resource networks.
• Facilitate community support groups that promote connection and healthy coping strategies.
• Assist clients in resolving urgent or emergency situations.
• Participate in trainings and ensure compliance with all organizational policies and procedures.
