Qualifications

• Minimum of one year work experience in health care.

• Ability to use Microsoft Office (Word, Excel, Outlook) preferred.

Responsibilities

• Track referrals and document release of information and referral documents.
• Register patients accurately with demographics and insurance information.
• Provide referral information to specialists per guidelines.
• Coordinate with insurance for prior approvals and financial information.
• Explain referral details to patients and address barriers to care.
• Serve as system navigator for patients and families; advocate as needed.
• Identify community resources and maintain relationships with providers.
• Ensure timely addressing of referrals and remind patients of appointments.
