Job summary

PAM Health, LLC is seeking a Clinical Navigator to be the face of our hospital, cultivating referral relationships and managing a defined territory. You will meet patients at their times of need, coordinate with admissions and nursing staff, and help drive successful referrals and admissions.

The role emphasizes collaboration, data analysis, and a focus on patient and source satisfaction, with opportunities for education advancement within our PAMily.

Qualifications

• Current licensure in nursing or respiratory therapy, or OT/PT/SLP.
• Minimum of two years' experience in the designated license field, preferably with rehabilitation/LTACH.
• Marketing experience preferred.

Responsibilities

• Cultivates referral relationships and manages assigned territory.
• Completes patient assessments and makes sales contacts.
• Analyzes referral and admission data with the team to facilitate conversions.
• Provides feedback for program development and quality improvement related to customer service and satisfaction.

Current licensure RN/OTR/PT/RRT/SLP
