Job summary

Enhabit Home Health & Hospice in Scottsdale is seeking a referrals coordinator to support patient referrals, serve as a resource for referral sources, and help coordinate transitions to services.

The role requires a high school diploma, 2+ years in health care and 1 year in home health or pediatrics, strong customer service, Medicare guidelines knowledge, and basic technology skills including mobile device use. This is a full-time on-site position with competitive benefits.

Qualifications
