Vivie is seeking a Referral Intake Coordinator in Minnesota to handle referrals, gather and verify client data, and ensure timely, accurate service delivery. You will manage intake data, coordinate with clinical teams, and support client inquiries with excellent service.

The role emphasizes EMR management, collaboration with Sales and HCBS teams, and attention to regulatory compliance. Strong organizational skills and English communication are essential for success in this on-site position.

Qualifikationen

• Strong communication skills and customer service orientation.
• Excellent organizational and data-entry accuracy.
• Experience with EMR systems and medical terminology is preferred.
• Able to pass state background checks and read/write/speak English clearly.

• Receive, review, and process referrals from physicians, hospitals, and other sources.
• Collect and verify intake data for accuracy and smooth handoffs to clinical teams.
• Field inbound calls and inquiries to assess needs and route clients to Vivie service lines.
• Maintain electronic health records securely and in compliance with regulations.
• Collaborate with Sales and HCBS staff to support care coordination and referrals.
• Support care delivery processes and participate in process improvements.
